ACC Application form
Use this form to request ACC approval of your project.
The Woodhaven HOA has Covenants, Conditions, and Restrictions (CC&R�s) that govern the mutual interests and property values of all owners. As a HOA member, you are obligated to comply with all community documents available at www.woodhavenhoa.org/documents. The Architectural Control Committee (ACC) must approve any and all exterior improvements and changes (including back yard renovations) prior to beginning work. ACC application forms are available at woodhavenhoa.org/ documents. Any work completed without ACC approval may result in conditions, changes, financial penalties, or removal of work. All construction must comply with local, state, and federal building and land use regulations. Compliance and permits are in addition to the Woodhaven ACC process and are the responsibility of the homeowner. Information is available on the City of Sherwood Planning and Building Department web pages. Changes made to ACC approved plans to comply with necessary permits require additional approval by the ACC. ACC applications will be considered by the ACC when the application is complete. Decisions may take up to 30 business days of the fully completed application. This includes, in addition to this application form, as appropriate: For landscaping and construction: Site plan drawing (to scale) of the design showing street(s), paths, lot, house, setbacks, additional structures, fences; rendering of proposed construction design (to scale), including finished dimensions, e.g., additions, fences, gates, arbors, and trellises, parking pads; signed letter of approval from adjacent neighbors if requesting to paint or stain fences, gates, etc; identification of types of materials to be used. For all exterior painting and staining of buildings, fences, arbors, etc: Color samples painted on the house, labeled and easily accessible for viewing; description and color of any brick or stone on the front facade of the house; paint chips matching sample colors on the house (Internet colors are not true and are not acceptable) received at Northwest Community Management.

Building, electrical, or plumbing permits may be required. The determination of the necessity of any permits is the responsibility of the homeowner. Obtaining any necessary permits is the responsibility of the homeowner. Changes to the proposed plan necessitated by permits or compliance with local, state, or federal building or land use laws are subject to additional review by the Architectural Control Committee (ACC).
